Our online feedback form is an easy way to send us your comments. There are "Tell Us How We're Doing" forms available at 333 East Campus Mall. Be sure to ask someone on staff if you can't find one. Drop your comments into one of the brown wood suggestion boxes. You can also give a completed form to any staffer at UHS, or return it free in the UW Campus Mail system.
