Where or how do I complete a health form for UW-Madison?
The form must be completed online: http://www.uhs.wisc.edu/display_story.jsp?id=995&cat_id=178 for more information.

Do not send us any paper records.

Is the form mandatory?
No, but completing it is strongly recommended. Completion of the form helps your clinician at UHS provide you with better health care based on the most complete information. It also provides you with documentation of your immunizations and other health data that you may need later for travel, employment or personal reasons.

Important note: Students who live in a university residence hall are required by Wisconsin state law to inform the university whether or not they have received hepatitis B vaccine and meningococcal vaccine. The UHS immunization and health history form is the way the university collects this information. Completion of the form will satisfy the legal requirement for students.

Can a parent or health care provider complete the form for the student?
Parents and health care providers can be very helpful resources to students as they fill out their health history. UHS strongly discourages anyone else from completing the form for the student, because access to a student’s health record should be limited to the student and his or her care providers, and we urge students not to give out their UW NetIDs and passwords to anyone for reasons of privacy and security.

Do I need to submit a physician signature or certification of my health form?
No.

Do I need a physical exam before coming to UW-Madison?
No.

Do I need any laboratory work done?
No.

What vaccines are required to enroll at UW-Madison?
No vaccinations are required for new students, although a number are strongly recommended. Certain schools in the health sciences do have requirements for students on clinical rotations. Please contact them for details if this applies to you.
