Do I need a physical exam before coming to campus? Do I need to bring my medical records? The university and UHS do not require a physical exam. We do, however, request some information about your health history before you arrive on campus, so that we have this information on hand should you need to visit UHS. Please complete the UHS Immunization and Health History Form to submit your information to us electronically. Please do not mail immunization records to us. Generally, you do not need to send or bring your medical records to UHS. However, if you have chronic or recurrent condition, treatment in process for an illness or injury, or any other need for continuity and communication between clinicians, copies of records can and should be sent or brought to the Medical Records Department, University Health Services, 8th floor, 333 East Campus Mall. Continuity of care is greatly enhanced by good communication between your clinicians at home and at UHS. Although the prices on many medications at the UHS Pharmacy are substantially reduced for students, standard coverage at UHS does not include prescriptions.
