Will the pharmacy at UHS fill prescriptions written by my doctor at home?

A:
Yes, the pharmacy at UHS will fill prescriptions written by any clinician licensed in the United States.

To transfer a prescription to the UHS pharmacy, download this form and fill in the information about your prescriptions. Send it to the pharmacy via fax (608-265-8714) or email (send the form as an attachment, and put "University Health Services" in the subject line). Allow one business day before picking up your prescription.

Cash, check, Visa, and MasterCard are accepted payment options. In addition, the pharmacy can accept cards from a number of health insurance plans. Please check with the pharmacy for the latest list (263-4990). You should have your own insurance identification card or a copy of a parent's card (both sides).
