Parents: Seven Things You Can Do When Your Student Heads Off to College

| Together, complete the online Immunization and Health History form with your student so we'll be prepared if your student needs to visit UHS during the semester. It's a good way for students to get more familiar with the health history of themselves and their family.

| Confirm that your student has received vaccinations for Hepatitis B (HBV), Measles-Mumps-Rubella (MMR), and Tetanus-Diphtheria (dT). Students should receive the meningococcal conjugate vaccine before arriving on campus. It is available from most health care providers. Students can also get the vaccine at UHS for a fee; call (608) 265‑5600 to schedule an appointment. More information about this vaccination is available here.

| Read our chart of services so you know what UHS provides to students as part of their tuition fees. If your student is not covered by your insurance, consider purchasing the SHIP Health Plan for comprehensive, university-sponsored medical coverage.

| If your student is included in your prescription or health insurance, make photocopies of both sides of all cards for them to bring to campus.

| Transfer prescriptions from your student's home clinic to the UHS pharmacy. It's closer to campus buildings than any other pharmacy, and many medications have special reduced student prices. Call the pharmacy directly at 608-263-4990.

| Put together a first-aid kit for your student to bring to their new residence. Consider band-aids, anti-bacterial cream, an electronic thermometer, tweezers, common remedies (e.g., ibuprofen, cough drops, diarrhea medicine, etc.). Include our phone number (608-265-5600) so they can call for an appointment if they need something more than the first-aid kit!

| Look over our "Notice of Privacy Practices" (pdf), which describes our confidentiality guidelines. Understand the limits of when UHS will share health information about students with their parents.
