The UW-Madison Student Health Insurance Plan for International Students

Safeguarding your health
SHIP, the Student Health Insurance Plan offered through University Health Services (UHS), provides international students with a comprehensive and accessible health insurance plan. Medical treatment in the United States is very expensive, and quality insurance coverage is essential. The health care system in the United States can also be difficult to navigate, and we are here for you if you have any questions.

Health insurance compliance
All international students and visa dependents are required to have UW–Madison approved health insurance coverage. You must enroll in SHIP or file a Waiver Application before the stated deadline — unless you qualify for an automatic waiver.

When is the compliance deadline?
Fall Deadline: September 15, 2009
Spring/Summer Deadline: February 15, 2010

We require full payment of the SHIP premium or a completed Waiver Application by these dates. Failure to meet the compliance deadline will result in a $50 late fee in addition to any required SHIP premiums.

How do I enroll in SHIP?
SHIP will accept Spring/Summer enrollments from registered students beginning December 15, 2009.



  • Enroll at the SHIP Office – 9am to 5pm, Monday - Friday(MasterCard/Visa/Check)

  • Enroll by telephone (608) 265-5232 – 9am to 5pm, Monday – Friday (MasterCard/VISA)


Parents can enroll their registered student in the SHIP office, by telephone, or by mail.

How do I file a Waiver Application?
Please read the Waiver Application carefully before you purchase other health insurance. Waivers will only be approved for plans that meet the criteria listed on the Waiver Application. Completed Waiver Applications must be received at the SHIP office by the stated deadline. If you do not qualify for a waiver you are required to enroll in SHIP.

Automatic Waivers
You do not need to file a Waiver Application if you are employed at UW–Madison and are receiving health insurance benefits that are effective on or before September 1, 2009, for fall or February 1, 2010, for spring. Dependents of UW–Madison employees who are also international students must file their own Waiver Application with the SHIP office. If you lose or change your UW–Madison health insurance benefits, you are required to enroll in SHIP within 30 days. Back to Top
