Prescriptions
It is very important to tell us of any medication allergies you have. Please allow at least one hour to ensure your prescriptions will be ready.
Refill a Prescription
You can refill prescriptions:
- Online;
- Or by calling 608-263-4990 and selecting option (1) to access the voicemail that can take your order even during the hours that UHS is closed.
- If refilling by phone, pleast tell us:
- Your name
- Your student ID number
- Phone number
- Prescription number (which is found in the upper right hand corner of your prescription label, highlighted in yellow and beginning with 03)
- Quantity of medicine you want
To minimize wait times, please call at least one business day in advance. The pharmacy voice mail can take your order even during the hours that UHS is closed.
Refilling a prescription filled at another pharmacy
Bring your medication container with that pharmacy’s label, along with your student ID card, insurance card(s), and payment. The UHS pharmacy will call the other pharmacy and ask to transfer the prescription for you.
Fill a New Prescription
Bring your prescription, your student ID card, and payment to the pharmacy. If your home-town health care provider would like to phone in a prescription for you, they can call the pharmacy at 608-263-4990 or fax it to us at 608-265-8714.
Transfer Prescriptions
To transfer a prescription to the UHS pharmacy, download this form and fill in the information about your prescriptions. Send it to the pharmacy via fax 608-265-8714 or email (send the form as an attachment, and put “University Health Services” in the subject line). Allow one business day before picking up your prescription.
Summer Pharmacy Mail-Out Service
For students who move away for the summer but will return to UW-Madison as a student in the fall, the pharmacy is happy to ship your medications to you instead of transferring your prescription to another pharmacy.
To request this service, call 608-263-4990 and select option (5). Please provide your name, shipping address, and Visa/MasterCard information. The cost is your regular prescription price plus a small shipping fee. Refrigerated or narcotic-type medications cannot be shipped. The pharmacy is unable to ship medications outside the continental United States.
