UHS provides ergonomic services free-of-charge to eligible UW–Madison employees to promote health and safety, decrease injury potential and Worker’s Compensation expenses, and increase productivity.

Service Requests

Schedule ergonomic showroom visit

1. Email to schedule a visit. Appointments are required.

No walk-ins are allowed.

2. Read the furniture memo (pdf) and review your unit’s purchasing policies.

The ergonomics program does not purchase or install furniture and equipment.

3. Find directions to the UHS Ergonomic Showroom for your scheduled appointment.

Request a workplace assessment

1. Obtain supervisor approval.

2. View appropriate prerequisite training(s).

3. Submit the appropriate symptom survey.

Request a virtual ergonomics consult

1. Complete and submit this form to request ergonomic services.  You may need a:

  • ruler or tape measure for workstation dimensions and a
  • camera or photo device with timer or another person to take photos.

2. Inform your supervisor or Divisional Disability Representative (DDR) of your request.

3. Contact your DDR (listed here) if you are filing a reasonable accomodation or have difficulties completing this request.
