A few reminders:
Come to campus with an adequate supply of medication until you establish care with a UHS provider. If you need to request psychiatric prescription from UHS—including ADHD medication— you must first be seen in person at UHS. Prior to that appointment, UHS must receive and review your medical records. To transfer medical records from a home provider, fill out a release of information. If you want to transfer ADHD or other psychiatric medications to UHS, schedule an Access Appointment early in the semester.
Immunization and Health History Form
Complete your Immunization and Health History Form in MyUHS. All new students entering UW-Madison must complete an Immunization and Health History Form. The information you provide is confidential and will be retained by UHS as part of a student’s medical record. This information is valuable to students as they receive health care throughout their UW-Madison career. The Immunization and Health History Form is only available online through a student’s MyUHS account.
