UW–Madison and University Health Services have certain health requirements that allow us to best serve each student’s individual health needs and identify and collect specific information related to campus communicable disease prevention.
University policy requires all students to share their vaccination status. Certain health forms are required to access UHS services. This page shares information on how to complete these requirements.
Share Your Vaccination Status
University policy requires students to share their vaccination status for the following communicable diseases through MedProctor:
- Measles, Mumps, Rubella (MMR)
- Tetanus, Diphtheria, Pertussis (Tdap)
- Varicella (Chicken Pox)
- Meningococcal (ACWY, B)
- Hepatitis B
This is not a requirement of vaccination—it is a requirement to share your vaccination status for these diseases. Students who do not complete this requirement are subject to a registration hold that will delay future course enrollment.
What You Need to Do
Incoming fall 2026 students must complete this requirement by August 7
- If you are vaccinated, upload your vaccination records through MedProctor.
- If you are not vaccinated or do not have proof of immunity, complete a brief attestation form through MedProctor.

Complete Required Health Forms
Complete required health forms in the “Medical Clearances” section of MyUHS. You will not be able to receive medical or mental health services at University Health Services without completing certain health forms through MyUHS.
Access your MyUHS account, and log in using your NetID and password. Incoming students for fall 2026 will have access to MyUHS beginning in mid-July. Click on “Medical Clearances” in the right sidebar to complete required items:

- Minor Consent Form (students under age 18)
- Health History

Information and Consent Form
This form is required to use UHS services. You will not be able to receive medical or mental health care at University Health Services without completing this form.
Minor Consent Form (students under age 18)
Students who are under the age of 18 as of August 15 must have a parent or legal guardian sign a consent form to be eligible to receive all UHS services. Without a signed minor consent form, many UHS medical and mental health services cannot be provided. STI testing, contraceptives, pregnancy testing, and sexual assault-related services are available to minors without parent/legal guardian consent.

Enter information on your medications, allergies, and health history. This information will help your UHS health providers better understand your health to provide the care you need.
